26R4 Is Live: 49M Patients, 514.8M Clinical Notes, Expanded NLP Coverage, and a Smarter Platform

October 1, 2026

NeuroBlu Analytics v5.4 is live with the new 26R4 dataset and several updates that make the platform easier to use.

The data: the dataset update brings the total number of patients to 49M, and the number of clinical notes past half a billion. 26R4 also features more depression and suicide-risk data pulled from notes, as well as early access to extracted information on medication changes, clinical rationale, and reported side effects.

The platform: v5.4 introduces a smarter search with an AI summary of the documentation, a more functional homepage, improved PHQ-9 filtering and overview in Cohort Explorer, and 14 new Code Studio functions.

26R4 Dataset: 49M Patients

26R4 adds more than 2.5M patients to the neurological cohort, bringing total coverage to 49M patients. Migraine grows to 4.7M patients, and Parkinson's disease and Alzheimer's disease both see strong gains.

Patient coverage across key neurological conditions now includes:

  • Pain Disorders (25.7M)
  • Migraine (4.7M)
  • Sleep Disorders (2.1M)
  • Dementia (2.1M)
  • Epilepsy (889K)
  • Alzheimer's Disease (635K)
  • Mild Cognitive Impairment (464K)
  • Parkinson's Disease (551K)
  • Narcolepsy (83.9K)
  • ALS (11.5K)

Our behavioral health dataset also continues to expand:

  • Schizoaffective Disorder (444K)
  • Borderline Personality Disorder (280K)

‍

Clinical Notes: 514.8M

26R4 adds 258.7M new clinical notes in a single release, bringing the total past half a billion to 514.8M.

‍Deeper NLP: Depressive Symptoms and Suicidality

The depressive disorder model coverage was expanded with clinical notes from a new data partner, capturing evidence of anhedonia, depressed mood, fatigue, negative view of self, and difficulty concentrating.

The suicidality model was refreshed with recent notes from a data partner, capturing evidence of suicidal thoughts, suicidal behavior, and suicide attempts.

Overall, these updates added around 1.6M patients with NLP-extracted evidence across both models. These labels turn free-text narrative into structured criteria you can filter on directly, so you can define depression and suicide-risk cohorts with detail that diagnosis codes don't capture.

MedManagement: Early Access Preview

Uncover the clinical context behind prescription updates.

Using Holmusk's proprietary Medication Management LLM, we extracted medication changes, clinical rationale, and reported side effects from neuropsychiatric notes into a standardized, structured table.

The table is now available as early access preview spanning 620k patients, giving researchers visibility into treatment dynamics and switching behavior.

What's New in the NeuroBlu Platform

Universal Search with AI Summary

Ask questions about the platform or the data and get a summary of our documentation alongside a full list of links to the sources. The search experience has also improved to return more close matches, so you can find what you need without knowing the exact term.

A Redesigned Homepage‍

Use cases, documentation, release updates, and tools now sit in one place, so every session starts with everything you need in reach.

PHQ-9 Item Filtering in Cohort Explorer‍

Define cohorts by PHQ-9 severity category or by individual item responses. Build a population around specific symptoms, like anhedonia or sleep disturbance, instead of total score.

‍Code Studio: 14 New Outcome-Measure Functions‍

Code Studio adds 14 new R and Python functions for outcome measures. Changes made in Cohort Explorer also sync to Code Studio faster, with a warning shown while sync is in progress.
